    they're away from home or in a new environment. This behavioral assessment helps us learn more about your pet's
    personality, routines, and past experiences so that we can provide the best care and make their stay as comfortable
    as possible.

    Please answer the following questions as accurately as you can:

    Is your dog crate/kennel trained?

    Has your dog ever attended daycare or stayed at a boarding facility before?

    If yes, how did they interact with others dogs and staff?

    How does your dog typically react to new people?

    How does your dog typically react to other dogs?

    Has your dog shown signs of aggression (growling, snapping, biting)?

    If yes, please describe the situation:

    Are there any specific triggers we should be aware of (restraint, confinement, toys, food)?

    Does your dog have any known fears or anxieties (thunderstorms, separation, etc.)?

    How has your dog typically behaved during past veterinary visits?

    Any additional information or special handling instructions?

    I, the undersigned owner, confirm that the information I have provided is true and accurate to the best of my
    knowledge. I understand that Lone Mountain Animal Hospital will rely on this information to ensure the safe and appropriate care of my pet during their stay. If my dog displays behavioral problems or experiences a medical or behavioral emergency, I agree to make arrangements for myself or an authorized person to promptly pick up my pet upon request from the staff. If my dog shows signs of stress or anxiety, and a licensed veterinarian determines that sedation is necessary, I authorize the staff of Lone Mountain Animal Hospital to administer oral sedatives to help keep my dog comfortable and reduce anxiety. I acknowledge that a medication administration fee will apply, in addition to the cost of the medication itself.
