To redeem your gift card, present the card at time of purchase. The amount will be placed on your hospital account and will be deducted from your purchases. Your gift card is not redeemable for cash or for purchases of other cards. The unused valueremains on your hospital account. Lone Mountain Animal Hospital is not responsible for lost, stolen or damaged cards or cards used without authorization. Cards cannot be replaced if lost or stolen.
Once your card is redeemed you may check your account balance during business hours by calling 702-645-3116. This card contains security features. If these features are not present, or appear to have been tampered with, Lone Mountain Animal Hospital reserves the right to only honor the card upon the production of a valid receipt from Lone Mountain Animal Hospital for the purchase of the card. Lone Mountain Animal Hospital reserves the right to change these requirements.
This card expires 24 months from the date of purchase.
